Because it's only coagulated. It hasn't attached to anything more than gelatin would inside his nasal cavity, and thus doesn't hurt coming out, since nothing is being pulled.
If anything it has a really odd sense of pleasure coming out. Hard to describe. Nowhere near painful though.
He doesn't need to see any type of doctor, this is NORMAL. It isn't like he's got some disease that's making string blood in his nose, it's just drying blood from a nose bleed.

That isn't really 'massive', and it isn't hard to have a big nosebleed. Your nose, especially farther up in the nasal cavity, is a moist place. That makes it a bit harder for the blood to clot and dry to stop the flow of blood due to mucus in the area.
