The problem was when imagery began to appear it ended instantly. Control yourself when the imagery appears, know that it's there but don't focus on them, otherwise you'll end up waking up. If you do get into sleep paralysis, remember that it's just a dream. The best way to get out of sleep paralysis is to wiggle your toes. After a while, you'll break free and you'll have full control of your body. Everything around that you once saw will disappear. If it helps any, close your eyes. Sometimes you can block the alternate reality, but most of the time, like mine, it'll just go through it and you'll still see them all. Of course that's no reason to break free of SP instantly. If you start seeing the creepy fuckers, break out. If you can maintain your eyes closed and paralysis enabled, you'll be steps away from lucidity.

When you get to the sleep paralysis stage, just relax and go with the flow, try not to get scared or excited. Imagine an ocean with several layers. When you're awake, you're swimming at the surface. And then you relax, become still and sink deeper, until you reach a certain depth which is sleep paralysis. If you start panicking and such, you're going to get stuck there. You won't sink deeper. What you need to do is keep relaxing, so you can sink past the SP stage to the next sea-layer below it that is the lucid dream.

Well guys i tried to lucid dream last night. I could feel it starting but i could never actually get into the dream. Like right when i was about to fall asleep i kept saying to myself i was dreaming. What was freaky as right before i fell asleep i heard someone scream my name twice... It was scared me.

In fact, I intentionally look at my hands at the very beginning of a lucid to make the dream more stable, clear and remove fuzziness. If I don't stabilize the dream with the hand looking method then it starts to fade away fuzzier and fuzzier and I wake up two seconds later.

Technically SP DOES happen when you're going to sleep, but that's what's called hypnagogic imagery. Which is completely normal. The freaky SP is when you wake up in S4 (see the graph in OP) or really deep sleep and your brain is still locked down and your brain's in dream mode. If SP didn't happen, you'd be reenacting your dreams, which is obviously bad because you'd probably kill yourself by accident or break your arm or something. You can do shit for months (not literally but you get my point), and then when you wake up you feel great and refreshed.
