Look the best you can do is just be there for him, I had a friend that started acting paraniod one day and then ... I started to lose him but luckily he went to a hospital and they found out what was wrong with him. Sadly he needs to be on meds for the rest of his life and worse he is a different person now. Just be there for him ... that's all you can do. If he does come out work with the parents and be there and don't walk away.
