I've actually had two (to my knowledge) near death experiences. The first was when I was five, I was in a swimming pool and my "friend" (who was also five) started trying to drown me with a inflatable boat. I was probably under water for a good two minutes.
The second time was when I was either six or seven; I was at a water park and was going down one of the slides (meant for tubes) and for some reason my tube flipped. Nothing serious really, I got back on it - but then some jackass was sliding down right behind me and slammed into my tube.
So basically, my tube went almost all the way off the slide (the top half was open) and I got a very nice view of the park below. I even felt the fucking tube start tipping over the side before gravity took hold and I was back on the slide. My hypothesis is, that if I hadn't already been moving and had even gone another 15th of an inch over, I would have fallen off the slide and to my death.
My friend started humping a lamp post and then I couldn't stop laughing and couldn't breathe.
I was riding my scooter through an unmarked and supposedly clear intersection until a police vehicle speeds through my side of the street and makes a turn right into me without sirens or lights and nearly clips my foot which would have effectively landed my face with some pretty asphalt.
As well, one time when I went snowboarding I lost control and was heading quickly towards a batch of trees face first until I decided to try to stop the fall by sliding my arms for more friction on the ground. Eventually my scaphoid couldn't take the pain and broke. Thank goodness there wasn't much snow under those trees.
pretty bad concussion in 3rd from trying to run away from some dickweed bully who was trying to take my ball or some shit, slid under a railing and hit my head and fainted, woke up in the hospital tired as fuck
A jumped off a 6 meter high surface onto the ground, but i flopped my landing and I hit my head on a steel bar, surprisingly I nerve felt anything but I was quite dazed.
I almost drowned in my uncle's pool. I was rescued by my cousin.
I was also hit by a car while I was in the crosswalk. The light was on my side when some idiot started to make a right turn without paying attention. I was literally only a few inches in front of her bumper. I got knocked into the street, but only had bruises.
Almost drowning in a swimming pool when I was younger. I used to be a really good swimmer, though I hadn't been swimming in years and I accidentally strayed into the deep end with my best friend. I swallowed a lot of water and got weak chlorine poisoning if I remember rightly (I've had it twice in my life, horrible thing to have), at the time I had to bob myself on the surface of the water and ended up grabbing one of those string-section dividers and pulled myself along it until the water was shallow enough for me to swim in.
It was a pretty stupid thing to do, but eh, I was young, and I learned from it.
I got a severe concussion on my ATV out in the middle of the woods when I was twelve. Apparently, I wandered two miles back to home. I don't remember anything besides constantly repeating the same phrase.
I ate a Big Mac
3 days ago me and my friend were bored. So we decided to walk alongside the train tracks. The tracks were enclosed by a fence, but there was a big hole on one side. So after 10 minutes of fucking around, my friend see's the fucking train. We bolted towards the hole in the fence, but we weren't fast enough. At the last second we jumped into a ditch. The train stopped and we ran away. Probably one of the stupidest things I've done too.

Swear to god I have never been so scared or tired in my life. I fainted twice on my way up because of the crash from adrenaline and pure exhaustion. I was really, really lucky that I had an awesome climber with me.
Sinus infection which broke out into the back of my eye, pushing it out with intense pressure. Without major surgery, I would have died.

Got hit by a speeding car that ran a red light. Hit while I was on foot, I mean.
Hit by a drunk driver and blacked out on the street, alone. Thankfully someone finally drove down the road soon after and found me face down near the gutter (was going to cross the road) Doctors say its a miracle that I didn't break any bones, only bruised them. Turns out that was the good part, bad part was nightmares, random blackouts and loss of vision at some times.
I was bouncing on my bed when I was like 5, smashed my head on a wall and had to go to hospital to get x rays and stuff. It left an egg sized bump behind.
When I was an idiotic little kid, I turned left with my bike without signalling that. So a car hit me and I flew off my bike. I only had a few bruises, and my bike was wrecked, but the car did not have any damage. Thank god the car is OK.
Almost drowned when I went swimming in extremely big waves.

Rode my bike down a hill, didn't know that the very sharp turn up ahead was icier than fuck and I flew off the bike and landed on my head.
Good fucking thing I had my helmet on, there was a 3 cm dent in it where I landed.
tried to open a chemistry pack with my teeth when I was seven, punctured a packet containing a sizeable amount of methane blue

I lost control of my car at 65mph and the backend went out, luckily right as it did, there was an average right turn coming up and I somehow skidded around it, the car then fishtailed about 5 times but I managed to keep it on the road.
Really isn't near death but I almost blinded one eye: When I was around 4/5/6 was playing in a blanket on the couch. Fell off the couch and and hit a spot about 1/3 of an inch away from my eye, on the temple side of my face. Shortly after went to the hospital and received stitches. Left a crescent moon shaped scar around my eye, very small though and not very noticeable now.

The feeling of shock blocks out pain.
Its why people crash their cars or bikes in a instant, and don't feel the pain for at least 10 seconds.
Hard to say, I fell off a 25 foot chairlift when I was 11, so I suppose that would count.
When I was 4 my mother took me on a raft and swam around the river and I got caught by a undertow and I almost drowned.
Cancer then car crash.
