I don't know about diarrhea, but when the mother pushes, she is using a lot of strength, and the whole body is trying to make room for the baby. It's not uncommon for a mother to shit herself when giving birth.

Yes happens a lot actually, the hormones and stress causes it, and a lot of women also throw up.
