His right side of his face is dropped about 10% and, when he's talking, you can see his right side of the lips is droopy and is partially responsible for his small speech blunders (long S's, slight lisp, sounds like he's got food in his mouth, etc)
Combined with possible drug/alcohol use causing [I]possible[/I] liver issues -

The whites of his eyes and skin both seem slightly yellowed compared to normal tinges.
Adding in the (rumored) stroke, I'd say he is possibly in bad shape. I hope he's alright, I've really liked his advertisements!

The guy obviously is a drug addict who funds his hobby by doing silly commercials. I can't see why it comes as a shock that his face looks like shit.

It's been one of those "lifehacks" for a decade to do that with a rock and the tubing you use for air in fishtanks.
It depends on how the tube is attached in the bottle though.
