Are his parents/direct family skinny as well? I'm bitch-ass skinny myself (47kg ~ 103.6 pounds), though a happy healthy boy. Odds are it's genetics.

It would help to know height vs. weight. I'm not huge myself, 178 cm and 57 kg (5'10", 125 lbs).
