Pat actually is currently in the middle of actually answering questions and berating clowns on twitter. He's been at it for a few hours now actually.
The sword is something he knew about because someone mentioned it in the SBF subreddit, and a lot of the other stuff was just shit he missed that someone did point out to him. Apart from the torches, I don't think he explained that.

No you don't understand Pat knew where an NPC was and now the lp is unwatchable.
I don't understand the mindset. I'm watching it because i like listening to their commentary.
Though him immediately knowing about the torch and the leeches was pretty baffling. I don't mind them getting informed about the NPC questlines and stuff (FROM made them extra extra bullshit this time around. I ruined an entire NPC for myself because I walked one room away from a bonfire in the wrong direction which was enough to break the chain), him clearly knowing certain mechanics inside and out is no fun.

The fact that using the dungeon bonfire before you talk to[sp]Seigward[/sp]in the Boreal Valley, ruins the questline is some serious bull. [i]Hopefully[/i] that's only a bug that and should get fixed.

Pat mentioned on twitter that Epicnamebro tweeted about the leech guys and the torch a while back. So he had an idea but wasn't completely sure.

Oh fuck that. I found my way to that bonfire before actually exploring most of the Boreal Valley. That sounds pretty bug-like but it could be legit, just the flags that move the quest along being set because "huh you shouldn't be here, got get gud elsewhere" is easier than tracking shit.
It's a shame Pat seems to be going off ahead on his own. He's the only one who really, really reacts to lore references because most of it just goes straight over Wollie and Matt...Matt's just being the best hype boy he can for the current badass on screen. Missing his first time reactions isn't super great.

In my blind play through I think I fucked every quest. Without going back and starting from highwall after every boss, there's just no way to know.

Yeah, most of the quests in 3 are pretty convoluted compared to 1, 2, and BB's. There's a lot of checks just to make sure you don't mess one up.
