Are interior parts of models not done entirely separately and then just placed inside the model with an attach modifier or something?

It really depends on what you're doing. You can make them separately but usually you might just hide the stuff surrounding the interior parts instead. If you're making a mouth for example you would just hide the head geometry when it's in the way.
