Obviously this kind of thing is frowned upon in society but what can a person do? Getting therapy of a sorts can be embarrassing and nobody wants to be labeled a 'fuck-up' or a 'freak'. It's an outlet to a persons behavior that cannot be rationalized in real life and if masturbating to little anime girls or cutting up women keeps them for expressing this in real life then leave them be.
This is exactly what's wrong with things that come to fetishes. We attempt to define what normal is.
I find fake rape hot. I find the idea of making somebody helpless and having your way with them attractive. However, I would never do the real thing. Ever. I have never forced a partner to do this with me, although I do it if they enjoy it, which gladly my current one does. Does it make me immoral and wrong for finding the helplessness of the person arousing? No, not at all.
Does it make me a rapist? Most definitely not. I have never raped someone and never will, but once we start assuming that people who are into rape or gore are rapists or people who rape, fuck, mutilate, etc. we're being silly. It's prejudice. The point was brought up that if we say people who are into gore are immoral, then people who play Grand Theft Auto are as well. I agree with this completely. Although it may not be the same sort of pleasure, it still is pleasure. And it is attained from something that you would never do in real life, but part of the fun is that it's a fantasy.
I'm not sure if I want off-topic at some points, but my tl;dr is that just because somebody's into it, doesn't mean they are, and if we start assuming this it's wrong and just causes a whole slew of bias and problems. You like what you like, nobody should have to justify it.

And I wouldn't say it's an outlet. Lolicon is fine, it's not real girls, doubt the same people would masturbate to real girls having sex honestly. Sexual fetishes don't determine personality or actions.
You've even said a fetish is usually developed during a very impressionable period of your life, your childhood. You can't expect the experiences that made you develop the fetish be isolated to only that sexual fetish. That's hilariously stupid.

I don't have any solid evidence other than observations from personal experiences. What I'm pretty much thinking isn't why someone has a gore fetish, but they got one since searching the roots will most likely find you the reason why.

Well yeah I don't think most people would kill or rape for any kind of porn, but I don't think most people that wouldn't now would before the internet, either. They'd probably just live without it, perhaps even unaware of it. Fetishes aren't drugs, the only thing you lose out of not having them is an opportunity to pleasure yourself. If you're willing to kill for that, then you'd probably be willing to do anything else for it as well. So really killing for pleasure would be impractical for anyone who could get off to something else.
