Just because the parent wants to give birth in an environment they're comfortable with, they are willing to risk the health of a newborn child. Home birthing is one thing (and is uncommon for a reason) but in the middle of a dirty river with insects and god knows what in the water?

I don't get this at all? Why do you think we invented hospitals and figured out what the best way is? Because alot of the times in the medieval ages, hell even now in parts of africa, people who gave births died alot and alot of child mortality.

Except maternal and infant mortality was very high back 300 years, you were lucky if you survived giving birth and that your child lived to see adulthood.

The degree of encephalization in humans is ridiculously high. Basically baby heads are way bigger than they should be compared to female reproductive organs. This creates an abnormally high level of mortality during birth among humans compared to other primates.
So no, we have died pretty regularly for the past 200 thousand years during birth.
