From my understanding it was started on December 30th or 31st of 2004. You'll occasionally see a 2004er such as [url=http://www.facepunch.com/members/12-RayvenQ]RayvenQ[/url] and [url=http://www.facepunch.com/members/73-Tentacle]Tentacle[/url] but they're kinda rare anymore.

They make small amounts depending on whether or not you choose to donate to a mod or all mods when you buy something on Boostar. The max that can go to mods is 50% of a purchase.

That would be a really good feature to have. If your not looking at the ticker within about 10 minuets of being mentioned, you'll never see the notification.
