I presume* the fadeout is a layer that fades over your view to simulate a fade to black, but with how close Joel was standing it caused her face to be rendered on top.

Facade (from what it says on wikipedia) basically has you interact with two AI that take what you type and process a response from it depending on the situation. For example, if you compliment the decorations, the woman will take it as positive or sarcastic depending on the mood. It's pretty neat, and it's won a few rewards, though I don't think anyone should find entertainment in it.
It's amazing how far Joel can push it without getting kicked out in Facade.

I don't know exactly when, but it was during the SMBX contest- someone modded sprites into Dio for a boss, but when Vinny got there he just called him Goku. Was a pretty funny scene.
