If anything, the fact that it was only created as a way to cause a shitstorm makes it even more likely that it's just a badly written, serious game, rather than a clever ploy to ironically include everything that'd make a game serious when the only thing pointing towards that are your own assumptions

I didn't watch the full thing initially, but I just did and after that 666 I know for a fact it's not serious. I had my doubts about it not being serious, but there's no way not a single person on the team at least chuckled while planning that.

Because there's no sex. Or nudity. Or highly-detailed gore that shows people's entrails wiggling around as you rip out their jaws and eyeballs. It's not even as detailed as Postal 2. Heads explode, throats get slit, people get shot, but it's the hokey sort of basic blood and gore. Only the game's context of mass-murdering without any hint of irony or intended satire 'earned it' the AO rating, which doesn't make sense considering how many other games out there do the same thing but with a little satire (or not in the case of games like Gears of War, which indulges in the curbstomps and other executions) and get away with M on a regular basis because of context alone.

Because the ESRB was afraid of this game actually dehumanizing people and at the time it was still controversial, they didn't want to look bad probably. That's my guess and I could be wrong.
I also agree with RikohZX, the controversy sparked an a AO rating because at the time people were calling this the spawn of inhumanity, the context of mass murder alone was what got it the AO rating but not the hundreds of other horrible bits of context that would [B]ACTUALLY[/B] land a game an AO rating. I would understand if it had the guy making love to dead bodies or it was just so vivid with its executions... but really it's just another longer version of the MW2 Airport shooting all over again with slightly better graphics and bad framerate.
In other words it is pretty silly.
