He's been known for subtly noclipping during certain parts of the game for added effect, such as climbing over certain obstacles. He probably activated noclip as soon as Nihilanth's level loaded so that way he wouldn't immediately start falling and therefore provided a better camera view for him to scream.

He has actually spoke to other characters before on one or two occasions in the series. The one I remember is him saying something to a scientist and the scientist telling him to shut up.

well he does interact with scientists and struggle to talk when underwater so I always figured it was him talking out loud to himself because he's kinda crazy
