I have to say though, the older brother is a bit of cunt for not turning it off. It sounds like they're trying to get footage for a doctor or something but come on. It's clearly making things harder on her when he's provoking a reaction by making it so obvious he's taping him.

Get rid of his video games and TV. Lock him in his room, only allowed to come out to use the toilet. Stop feeding him junk food. It might help.
