I'm not about to go pro-ME:A (because my god), but I'm running a custom male Ryder and his facial animations were never ever this terrible.
His voice and lines are also much much better than female Ryder's.

The general consensus seems to be that the defaults are boring and awful faces, and that customs get better animations. The problem with Sara Ryder customs though is that the presets all look a bit fugly in their own way. Combine that with downgraded customization from the previous games and some people have been resorting to using the glitch where you customize Sara, back out and then pick the default, resulting in the preset head and better animations with the default's hair and skin.

Just look at the gun in the thumbnail, somehow a really shitty LOD model became top LOD. The lighting was just straight up removed. The props got worse, the art direction went from serious si-fi to yellow submarine on a bad day. Somehow the krogan was attacked by an avaunt guard makeup artist and had his materials butchered. And somehow they fucked up rain drops. I don't even know how you can do that with substance designer.

tbh the sad thing is, that even with the downgrade witcher 3 received, the game is still one of the prettiest games on the market today.
But then you look at this and it goes from pretty amazing to pretty much 5-6 years out of date

I've got a custom male Ryder too and his facial animations aren't horrible. Still got those dead eyes that occasionally clip through his eyelids though, but that pretty much applies to everyone in the game. :v:

But faces are probably the hardest thing to get right in 3d, and animation is probably the most brutal discipline behind it. You don't give out serious roles to randys with no experience, so that's still a symptom of ingroup bullshit.) and why they felt the need to tear down years of AAA level work with awful shit that is objectively worse in every possible regard than the previously locked in assets.
When you're in the final parts of game development, you don't go "hey you know what we need to do? Redo all the character models, facial animations, mocap, choreography, lighting, particles and voice acting" unless there's some weird resentful bullshit going on. you're mostly just desperately trying to put out every single fire that sprouts up and trying to wrangle your product into a finished state.
Now that's not to say that it's the single sole factor in this trainwreck, but i'd put money on it being among the biggest.

No, on face rigs that complicated, there is no automatic face animation. And all studios will have an internal standard rig or sculpt base so that all assets from all games can share a majority of the animation assets. So that they have a huge pool of animations/assets to draw from as time goes on, so that all artists know what they're working with at all times, so that no model gives peculiar problems or breaks in a novel way and so that that kind of "oh, we changed something and now a years worth of work just broke and we will never get it back" situation can never happen. The only explanation for that kind of drop is that at some point they went "welp, all this shit has to go" and threw everything out to start over. How late in and why, god only knows.

This is basically the maximum level of complexity where you can have automated facial animation.
Also i'd put any money on the horrible drop in model/material/lighting quality on them not being able to optimize the game in time, and them just ratcheting down the global LOD settings universally and going "There, fixed it". The art direction and animation is still a grand old incompetent mess though.

I may not be exactly knowledgable in modeling, animating and so forth, but it feels like at some point in development, a lot of the game's assets were probably modified significantly, but all the animations they'd had were broken in the process. The procedural lip sync is way more goofy and seemingly unfinished than the previous games, and most characters have absolutely no expression unless they're a major character that gets hand-animated cutscenes (aka the Ryders).
It's just a guess though.

It's easy to blame QA for bugs that are still in the game, but if normal people are just running into them without actively searching for them you can bet your ass testers found them as well. It's still up to the devs to actually fix them.

My guess from a technical perspective is that Bioware jumped the shark technologically speaking. Either they downgraded the final product so that there is uniformity between PC/Xbone/PS4, or they tried to go too far with the enviromental rendering and were forced to cut frames in animation or drop some entirely (you cant tell in screenshots anyhow right?)
You wouldn't expect NPC characters to be popping-in in a game released in 2017 unless they were taking up large amounts of resources. I have no evidence to support this suspicion, but it feels to me like ME:A was made by people who specialized in multiplayer gameplay and had to learn to make a single-player experience. Who better than MP specialists to make maps and abilities that work around free-form combat areas, rather than the linear corridors of Mass Effect Past.

Because they had a massive budget and 5 years to make this game. Add that with lessons that they should have learned from ME3, which makes MEA all the more troubling. They had every resource available to them.
