The way they move and the noises they make are really unsettling to me.
Running around multiplayer maps by yourself. Particularly in Source / Gold Src games. There's something really unsettling about it, like I'm being watched or something. It doesn't matter that I know for a fact that there's nothing going on, the feeling is constant and persistent. I can't really explain it.
Limbos, there is that endless, empty space that surrounds maps, when I was younger I was kinda scared of those. I thought that there were some kind of collosal, flying creatures lurking around over there. :v Today, I'm scared of huge water containers, thought of gigantic sea serpent lurking somewhere under the water makes me paranoid as fuck.

there was this one time about 10 years ago i was building in gmod alone in singleplayer on flatgrass, and i had that same unnerving feeling
and then suddenly all the doors i had made in my base just opened up on their own, and god damn, i have never alt f4'd out of a game faster in my life

I remember Jabu Jabu's Belly from Ocarina of Time and the moon from Majora's Mask haunting my childhood, much to the point where it was difficult to play them past certain points
This. I still can't finish hl1 because of that, had a bad trauma because of the Ichthyosaur when i was young.

Nowadays it's not a problem anymore, but back when I was young, I was scared shitless of the screen you'd get when you didn't insert a CD into your Gamecube.
The music and presentation of it made it feel like I just entered some forbidden dimension, not meant to be viewed by human eyes.
I also once got scared shitless when a Gamecube game crashed and emitted a noise as if someone was drilling his way out of my television.

In FF7 at the start when you have the timer going off to get out, that would scare the fuck out of me I couldn't ever get past without help. I don't know why but I wouldn't ever save the game so I would have to restart and fuck me I hated the start so much.

Anything to do with deep water or swimming where vision is obscured. It's fucking terrifying even though I really want to buy Sea of Thieves.
Deep, dark regions in any game. Silent enemies in those dark regions. Just the atmosphere of chilling silence, not knowing what's around the corner, and not knowing whether or not you're alone can be frightening.
That, or tedious and repetitive missions in games scare me into putting off those specific games entirely.

Also, I have not played super mario bros 3 in a very very long time. had it on the all stars cartridge.
But as a kid, I couldn't figure out the best way to get over a chain chomp, and someone thought it would be funny to have these jump out at the camera if you stood still for too long

I had ecco the dolphin as a kid. I wasn't as good at games back then so I'd use cheat codes pretty often. Turns out if you have invincibility and get eaten by the vortex queen this happens

That little grotto where nessie is in sm64ds. I saw it in the distance and for some reason assumed it was an enemy, and really didn't want to deal with that, so I handed it off to a friend and made him play that part for me. Didn't find out the truth until years later.

The only game I found even a bit scary that I played when I was young enough to get traumatized by them was the original Max Payne. I was like 12 when it came out and got it almost on release day.
The nightmare-scenes fucked me up, I always had to at least mute the sound. I especially remember the one where the babys corpse under the blanket waves at you.
I used the action replay thing a lot when I played pokemon on the DS. It was fun to walk around places you shouldn't be, until you go out of bounds...The Mystery Zone was absolutely terrifying to me. Something about an endless black void hidden in a fun light hearted pokemon game really fucked me up. Pretty sure I had some nightmares about running around in it endlessly. I still get anxious hearing this.
I was eight years old when I first played through Halo CE. The flood scared the crap outta me.
