I played unhealthy amount of STALKERs + mods, but still these friggin Bloodsuckers (tentacle-faced things) terrify me even at the daylight. At night, when you trawl back to the trader, with all that loot, and no stamina, their sudden appearance give you instant heart attack.
See, now i'm reminded of stuff like the "Ghost of Lockout" from Halo 2 and the early days of youtube.
The Sonic CD sound test easter egg is the scariest fucking thing I've seen. I hate it. Every time I think about it I shudder. https://youtu.be/brLsUgvGq9U
Headcrabs aren't scary. They really aren't. But after being hit with a poison one that I was ready to just get hit by and swing at for the first time at around 13, and seeing my health to go to 1hp, I freaked and developed a fear
Holy shit me too, I was death scared of the mummies patrolling around their house, and I had to get my stepfather to do the part where you walk up to the closet in the basement and Pamela's father comes out
https://youtu.be/h2J0EPGcfAc?t=4 this fucking level man, this level. this level gave me crippling paranoia for the rest of my life
https://youtu.be/Fo7HA7Y89XI This in the F.E.A.R demo. I almost shit my pants and threw my headphones to floor. I was already on the edge from the earlier scares of the demo. Later i told my friend to play this ending of the demo as i went under the blanket peaking as he played. He was also so scared that he didnt even watch the screen and smashed the mouse shooting the walls.
I have no fears in video game. The spook is short lived in all cases and transforms into blood for the blood god, after which all enemies are brutally exterminated or skipped completely because they're annoying cunts or i have low health (looking at you, fast spitting thingies from Duke Nukem 3D that have way too much health and deal way too much damage and leave toxic goo on floor when dead) The worse it gets is paranoia in them tense games like that hl2 mod with the island that you only have 1 part

Mysterious Ghost People in Fallout: New Vegas. https://vignette.wikia.nocookie.net/fallout/images/a/aa/Ghost_harvester.png/revision/latest/scale-to-width-down/333?cb=20110223144719
Majora's Mask and the redeads have been mentioned a few times, but the Happy Mask Salesman in MM really creeped me out as a kid. The way his animation just skips around is creepy enough, but what really takes the cake is when he learns you didn't get his mask back after the first cycle. The way his face just snaps to anger and he starts strangling Link by the throat (or so it looks) made me look away during that cutscene.
Minding my own damn business in New Vegas and getting suddendly jumped upon by the following https://files.facepunch.com/forum/upload/225139/bc5fc747-c71c-415d-b7e5-c8772a3345e1/64618-1519778445-104147681.jpeg

Subnautica scares me, but my fear of underwater stuff comes from Half-Life https://unknownworlds.com/subnautica/files/2016/05/crabsquid.jpg
Speaking of fallout 3, the centaurs. My first playthrough, never seen a centaur before, I was going under a bridge and I hit the vats key, and the camera spins me 180 degrees around, and I'm face to face with this... thing, and there's just the sickening realization, this used to be a human. It's moments like those that stick with you.
Now when I was a wee lil' shit I hid behind the couch and peeked my head around every now and then while my bother played Resident Evil. There was also F.E.A.R. which never actually scared me outside of one moment. It's been to long, but it was when those stealth guys first show up. Walkin' around those offices low on health and ammo and suddenly all the windows start breaking and the blinds flail. I had to sit in an open area with my back to a railing and the few shotgun rounds I had left straining my eyes to see the dots of their eyes glowing. I'm sure if I went back and played it, it would seem silly now. Then there's the ,I'm ashamed to admit, moment of genuine gnawing fear while I was playing Gmod on singleplayer of all things. So here I am, normal day of playing gmod building shit, havin' epic battles, posing ragdolls, all on the regular old Construct (I think this was in Gmod 9) So I get done killin' a hoard of zombies with willy contraptions, when I'm suddenly filled with this overwhelming sense that somethings watching me, I'm in a singleplayer game so I got up and looked around thinking maybe my mom or brother tried to tell me something and I just didn't notice. Nope, nothing, whole house is off doing their own things in their own rooms. So I think weird but whatever, put it out of my mind and go back to playing. Cept' I couldn't, whole rest of the day I felt those unseen eyes on the back of my head, got so bad I would walk around in game and check all the corners to see if maybe I accidentally hosted a server and someone was in here messing with me. Started thinking those zombie corpses weren't there before, that tub car was shifted over too much. Then the game nearly made me shit myself. This whole time I'm creeped out running around the map looking for signs of something when all of a sudden there are zombies walking around the hanger where I had staged the massacre. Thought it was weird they only creeped out of some room now after all this time, but whatever it'll get my mind off things. I whip out some weapons and I start mowin' down the zombies, all is going well, I'm standing at the big hanger entrance and I just got done gunnin' em' down, and before I can even turn around a goddamn antlion guard comes up behind me and kills me. I nearly shit myself. I paused the game(which did indeed pause it) and had to walk around my room for a moment, after which I respawned, killed the guard, and preceded to do one more round across the map. Now, I had already been around the map several times by that point, and I still never recall spawning an antlion guard even once, nor had I encountered it before killing the remaining zombies. I turned the game off, turned my computer off, and read a book the rest of the day, still feeling like somethings watching me. I didn't play Gmod again for about a month.
I can deal with the deep caves and such, but the crash site?
