Just to clarify things you said (in regards to how Melee, though Brawl follows the same guidelines):
-There is a criteria for stages. It can't significantly favor a character, have a "loop" in it (allowing a player to run away and stall the timer), have random elements that could kill someone (Bullet Bills on Peach's Castle), and it can't have walk off blast zones.
-Legal Stages: Final Destination, Battlefield, Yoshi's Story, Dreamland 64, Fountain of Dreams, sometimes Pokémon Stadium and/or Kongo Jungle 64.
Stages that can be counterpicked: Brinstar, Rainbow Cruise. Sometimes Mute City, Poké Floats, Pokémon Stadium, Kongo Jungle 64, Corneria.
-None of the characters are banned, ever. There never has been any community-wide character ban.
-All items are banned because they spawn randomly and can therefore kill someone randomly not mention their locations are random, giving a random advantage.
Not that anyone cares...
