Pre-invite: You could download and set up an account for free: this would allow you streaming access to everything Spotify had available, with the downside of and advert or two every 20 or so songs, which partially financed all the record licenses. You could upgrade to a premium account for £9.99, which got you new releases earlier, plus no ads. That was the other way of financing the licenses.
Post-invite: To get a free account, someone with a premium account needs to invite you. Premium accounts allow offline playlists, iPhone/Android usage, greater streaming bandwidth/higher fidelity, amongst other things. My guess is that they weren't making enough money to make ends meet, hence the restriction of free accounts.
As for the country question, some songs are unavailable depending on region, but that's usually only temporary: most the time, they can get it available worldwide.
