Not all of the fish though, just that huge one in particular. Especially how it fucking exploded into a swarm of normal fish when you shot it, I have a fucking phobia of swarms.
The Penumbra games. I bought it for $5 during a Steam sale a couple years ago and I still haven't gotten far in any of them.

I was never bothered by the ReDeads or Gibdos in Ocarina of Time and Majora's Mask, but what always scared me were the fucking Skulltulas, due entirely to my fear of spiders.

Pretty much all monsters from Return to Castle Wolfenstein scared me shitless, I still don't like them at all.
I am and have always been afraid of the STALKER games aswell, I had a fucking heart attack when I met my first bloodsucker and controllers freak me the fuck out. Still enjoy the games though.
Berserkers in Gears, any enemy in MGS (hate the alarm sound they make)... Mr Friend from Rugrats Search for Reptar. I'd avoid doing that level as a kid. :S
Oh and that big gorilla in Rugrats as well, in the toy store level.

Someone mentioned this on page 1, but Regenerators from RE4 and the bloodsuckers from Stalker take the cake for me, and Penumbra dogs. (And to think, all the Regenerators wanted was to hug me)

I farmed those guys like crazy to level up my materia to max level and cash it in for golden materia. I put about 300 hours into FFVII back when I had nothing better to do in high school.
