Actually Mal's totem would work fine for him.
BUT, what if it was all Mal's dream? Because since it was Mal's totem, it would function normally in her dream. Even if she is "dead" in the main storyline.
The whole film could've been her dream, and in reality she's still alive and well with Cobb.
Or alternatively, it could've been the last things that went through her mind as she was about to hit the pavement.

The top could also be symbolic of the nature of dreams themselves. A dream normally begins, and then ends. But while it's progressing (spinning), you don't know it's really a dream. It's only when you wake up (topple) do you realize that you were dreaming. But if your dream continues indefinitely (perpetual spinning), who's to say that's not your reality? And that "eternal dream" could represent death. You would never have woken up (toppled) to know that you weren't in reality.
