Actually, it was a closed fist to spare the gladiator. The thumb represented a sword, so thumbs-up implied a drawn sword (kill 'em) whereas a fist represented a sheathed sword (spare 'em).

I remember seeing somewhere that they never did that anyways. It was based on a painting (done ages after the Roman empire fell, of course,) which showed them merely pointing the thumb outwards.
