It's more of dropping somebody in at the deep end, by making them do actual procedures. They start you off small, with things like appendectomies and hernial repairs, and make you assist during major surgeries. They make you practice on gelatin models to get used to surgical technique like taking stitches and making/sewing up incisions, and use model laparoscopic devices to teach you a minimum on how to operate them. Most don't have the fine skill needed for that, it is really hard shit. The thing is, all of this only happens from the internship onwards, until the final year's end/internship you're not permitted to actually do any of this stuff, only observe and learn except by practicing on the models and gelatin blocks

Everything comes with practice against people slightly better than you but not overwhelmingly better than you. You could also watch people like Muselk or any other popular TF2er(Jerma included but he's a little on the silly side).
