If you're doing hard exercise, it's supposed to go up high - there's nothing wrong with that. If you're not running very fast at that point, you're in bad shape, but there's nothing inherently wrong with having a high pulse when training.

It doesn't skip a beat -- it's actually an extra beat between rhythm that makes it feel like your heart pauses for a second, you're having palpitations, and it may feel off rhythm. I'd definitely see a doctor about that.
