Bioshock was pretty scary. I knew I was in for a scary game when I was playing it for the first time at about midnight and that door shut on me once you enter that building.
The original Sims game did.
Metal Gear Solid 3, River of the Dead. Playing that at 2am and getting hit by one of The Sorrow's attacks scared the crap out of me.
Hitman: Contracts. Either the part where you're in the lab and there's people hanging from chains, or the glitch with the ghost.
When I was 6 I played Resident Evil 2 for the first time, there was a scene where you encounter the first Licker in the game and it crawled past the window, I saw it cross the window and I started crying and that game haunted me for years. I would always sleep with a night light and completely under the covers because I believed that would save me from flesh eating xombies.
Ahmaigawd. Also, on FEAR 2. I mean, I roamed the game for like 40 minutes in the building. I was paranoid that something would happen at any minute... I screamed more than once. In pure daylight.

Monsters in games don't really scare me. Getting utterly destroyed when I least expect it scares the shit out of me. L4D/L4D2 is good at that.

When I was like 6 or so, I'd go to this place which had Nintendo 64 set up for the kids. Zelda: Ocarina of Time was the game they had, so I'd play it and be like "Whoa this is cool Ima swingin' a sword around" and then I would leave the forest(the starting area) and go outside. So I'd be running around hyrule, until night came. That's when skeletons start popping out of the ground and I would totally flip out... Then, a few years back, I bought the game on the wii's virtual console. Now I have no idea why I was so scared. Right now if there was anything I would find near scary/disturbing it'd be the flood from halo. Not terrifying, but it just creeps me out a little.

When I first played Ravenholm and saw that torso hanging from the lamp at the very beginning, I skipped the level, instantly. Just went onto the next chapter. I did that for a few months before finally manning up.

Playing the original Doom when I was ... 5? Entered a dark room, and started getting attacked from no where. I almost pooped myself.
When I was playing bioshock a thuggish splicer appeared behind me in the dentists office I screamed and blew his legs off with the shotgun of course
FEAR was scary for me. Although I definitely think the final US mission in Battlezone was scary. Most intense mission in gaming history.
FEAR 1 made me shit myself IRL and made me look behind every once in 10 minutes. Doom 3 , fucking shit, too many scares, especialy the tub mutant, I shat myself. Area 51, multiplayer, when I was playing MeltDown map, I was just running around throwing grenades, when out of nowhere a mutant bitch appears in my face and melees the fuck out of me.:< Carnivores 1 , when I was 5 every fucking Trex, Velociraptor, *insertdinonamehere* attack made me fear, seriously, it was like, walk walk walk, and then GEGAAAWFAWFLFLFLFFL and they start to shake you. Gosh. Oh I remember more : Morrowind GOTY : When you leave the first city, that freaking rat/reptile, I didn't even saw it I was like just walking around and out of nowhere my screen gets red and I start jumping and knifing. :< I died too. Penumbra : I shat brix the whole gameplay...so yeah.
Fighting a tank for the first time in l4d. I was 14, yet almost shitting myself. Also Dead Rising when I was 8 at night. God the gargled moaning when they are idle. I was playing the Wii version last month, and almost shat myself when I opened the elevator to see 8 zombies run and latch onto me, ripping open my guts. Died in 3 seconds. 9 zombies on Hard diff' > 185 hp
Uh... Ravenholm in Half-Life 2. Terrifying chapter when you don't know how use the flashlight (yes, I was and still am an Idiot).

Doom 3
Dead Space. Its seriously creepy and freaky when you first play it, but eventually you get used to it. Was playing it on the 360 with a friend, and then we heard this THUNK.. THUNK.. THUNK.. THUNK.. and were pretty scared to go around the corner, thinking a boss or something was nearby. Then we see the guy smashing his head on the wall and just have a huge sigh of relief. Oh, and the huge evil regenerating boss, especially when you have to survive against it. We shat ourselves at the death scene for it.
