Fallout 3, a Radscorpion snuck up on my friend while they were playing, I was cracking some brazil nuts, and I got such a shock that I bent the metal nutcracker out of shape. Damn, every little scary thing in games terrifies me.
The only part of any game with the distinction of ever being properly menacing to me was back in the day when Ocarina of Time came out on the N64, and I encountered ReDeads for the first time. [img]http://images4.wikia.nocookie.net/zelda/images/a/a2/ReDead_%28Ocarina_of_Time_&_Majora%27s_Mask%29.png[/img] Now I had no idea what these evil motherfuckers were when I first saw them, but rest assured they shamble and groan in a way that makes it immediately clear to a small kid that it's a good idea to keep your distance. Trouble is there are a whole bunch of these bad boys skulking between where I am and where I wanted to explore. I twat the nearest one in its horrible, withered face a few times with the slingshot to see what happens, with the same morbid unease with which a kid might gingerly poke at a dead bird with a long twig. No reaction; it remains lethargically in place, apparently oblivious to the intrusion. Alright, I think, perhaps I can slip through without them noticing me. Here goes. Then the screaming starts. Screaming of the ghastly, blood-curdling variety that freezes you dead on the spot as the disfigured corpse fixes you with its hollow gaze and lurches forward. My stomach sinks as I realise the trap is sprung. I wrench the control stick with white knuckles and fumble desperately at the buttons in a vain attempt to free myself, but the hideous thing is now far too close to avoid and I can only watch on in silent, terrible, helpless panic as it proceeds to -[i]oh sweet jesus what is it doing?[/i]- as it proceeds to [b][i]hump my back and chew my fucking neck open[/i][/b]. Power off. Lesson learned. These days I've played through most of the decent scare-fests available on the PC, lights off and headphones on. I can honestly say nothing has come remotely close. The joys of childhood eh.
The Thunders 2 Alpha Beta when you got a army of BAC's running behind you. but when you hide in a room. THEY FUCKING SMASH THE WALL. then they come in and smash you to the ground and put a bag over your head.. god. i thought Dead Space and F.E.A.R where scary.. :Dawkins102:

And at the end of Penumbra: Overture :suicide: [sp]After killing the man who has saved you throughout the entire game, you go down a spiral staircase. You walk down, get to a long corridor. There's nothing there. You get 5 or 6 ft down, the lights all turn off. You try to turn on your flash light? No luck sir, doesn't work. So you're left with your glowstick which doesn't really do much lighting in the absolute dark. Walk a few more feet, the lights come back on, a big skulking figure at the end of the hall. You walk towards him. Lights start going off again, big systematically from the end of the hall. You walk down, you meet a door. try to open it, it's locked. Move on to the next. Try to open it. SMASH. The guy swings the door on your face, knocking you out.[/sp] And the game ends there, Black Plague picking it up directly from there. Oh, and Nosferatu. Fuck, that game was scary. I never even met a vampire. I start infront of the castle, pick up a sword and a revolver. Walk through a side door, the main door is locked. Walking up the spiral staircase, and I walk into a small room. Nothing there, just a fireplace. I walk in, seeing if there's anything there. Suddenly out of nowhere, a ghost comes out the fireplace, a skeleton out a dark corner. I shit it, run down the stairs. Skeleton sprinting after me, I fire my six shots, don't kill it, so I'm running as fast as I can down the stairs. I quit.
Doom 1 With Sounds as loud you can put them
I dunno, some of the underground levels in STALKER were pretty fucking terrifying. Doom 3 scared me to death when I was like 10, in the cutscene where that person's head explodes and shit flies out of it. The Penumbra games are great.

Doom 3 scared me shitless the first time I played it, but I was like 9 at the time. Since then, no game has really scared me much, but I did freak out a little on Penumbra: Overture.
