Five Nights at Freddy's (Before it got super duper popular and the "fanbase" ruined it). I mean the pure look of the game was just creepy, and the designs of the Animatronics were just legit unnerving. The very very very very worst of it all was the famous "Bonnie Face" thing. I'm thinking about it now and it's actually making my hair stand a bit.
Oh, and how can I forget the classic "I Have No Mouth and I Must Scream".

"I Have No Mouth and I Must Scream." Only game to ever disturb me and make me go "this shit is fucked what the hell." I mean GTAIV has that insane pedophile serial killer that freaked me out a little bit, and F.E.A.R. 2 gave me actual physical claustrophobia, but I don't think either of those really fit. Like, what the hell, killing the world and putting the survivors in their own personal hell for eternity? I actually fucking shuddered.
The only game that unsettled me so far was Metro 2033. [sp]The only part where I genuinely went 'fuck it, let's put up some music to bash through this' was in the Cursed station when you have to close the barricade and blow up the tunnel.[/sp]

[sp]It involves a group of bandits holding a woman hostage.[/sp] Unnerving in the fucked up sense, rather than creepy sense.
Solaris Temple's design is one of the few levels that while designed well for a game in a narrative point of view, comes off annoying and hard to view personally in a game which is a fast paced arpg. Probably would have been better as a prelude level without active enemies before the boss to make yourself filled with anger to defeat the boss. [t]http://i.imgur.com/WrYJPKql.jpg[/t]

It really caught me off guard when I first explored it.

I always liked Kelly and wanted my Shepard to bang her, so I quickly went from relieved to downright horrified when I saw her.[/sp]
I just remembered [sp]Jack Lupino[/sp] in Max Payne. Ragna Rock itself didn't creep me out, but the part where you enter [sp]Lupino's den, where you see the people Lupino sacrificed while he's shouting about "The flesh of fallen angels"[/sp] is when I start feeling uneasy.

The one thing that still creeps me out about Saints Row 3 is all those horrifying torture sounds randomly dotted around Steelport. What comes to mind for me is the electrocution house on Arapice Island and the guy getting his neck snapped in the Hideaway Hotel. Just ambient sounds like that in general make me feel uneasy. There's also the funeral home in Saints Row 2 and building 9A at the Boilingbroke Penitentiary in GTA V. Shivers my quivers every time.
[sp]Fresh Meat[/sp] in GTA V. If you [sp] don't arrive to the meat factory in time as Franklin [/sp], it triggers a cutscene where you see [sp] Michael [/sp] getting moved into the machine and then killed. Plus when you shoot at enemies in that mission,[sp] they don't die, they get knocked down and then they get killed by the machinery or fall into boiling chemicals and they scream in pain, it kinda disturbed me the first time.[/sp]

I had a fear of mounted deers on walls as a kid because of this fucker lol I couldn't play the story any more on my save file. I got my dad(who beat it on his) to beat it for me. Looking at it now, it doesn't look scary at all anymore due to old graphics, but jesus christ as a kid this was basically satan
For me it'd be either Amnesia or my first run through the Whiskey Hotel mission on MW2.

And that even though it was already rocking that "comic look". Then they killed the good but dark story with warcraft3 and afterwards ultimately with WoW.
