Some of my favorite zones are the grimdark ones. (EPL / WPL, Outland Shadowmoon Valley, Tirisfal Glades, etc.)
The ending of MGS:GZ where [sp]you see the bomb inside of Paz getting taken out without anesthetic.[/sp]. Like, a lot of the time I could handle that... but that just made me feel sick. After looking into the story a bit more it made me feel even sicker.

I think when most people see this kind of thing in media their immediate response is to shit on the artist because of how terrible and offensive the material is, but that's the point, you're supposed to look at these scenarios and think, "This is fucking disgusting. Is shit like this really happening in the world? This isn't right". He also handled it in a very tasteful way using only audio. It was hard to bear just listening to it without the need for any intense visuals. There's a tasteful way of freaking out your audience and then there's the dumbass shock-value shit seen in stuff like Hatred.
Sound design in games rather than any visual stuff is probably the only thing that could get to me in Video Games. Fantastic examples like the Ocean View Hotel in Vampire, the creepy corridors on board the Ishimura in Dead Space and this: [video=youtube;0NNwb6jg7PI]http://www.youtube.com/watch?v=0NNwb6jg7PI&index=23&list=PLOEAs04auvietkxFG-8hjc80IIVBItX2r[/video] It starts quiet but becomes louder towards the end, in the game itself it triggers itself into the next track if you encounter one of those creepy nurses, that track is terrifying high intensity industrial banging noise, it makes you panic like a madman. I had the Penumbra freeware on a cd, now I don't wanna hear or see anything about that series anymore.
Clive Barker's Undying genuinely unsettled me at times. One of the most atmospheric horror games I've had the chance of playing.

Super Mario 64 and its motherfucking killer piano. The entire Haunted mansion level always scared me as a kid.
I've mostly just seen Let's Plays of horror games (I'll play the Silent Hill series when I know I can have the time to dedicate to it) so I'm not counting any if the fucked up shit in those. Most of which has already been said. Of what I've played myself, anything to do with Caesar's Legion and the Fiends in Fallout: New Vegas disturbs me to no end. Even worse is all the Vulpes Inculta fanfiction, ugh. No thank you, Captain Sunshine. But luckily the game gives you snioer rifles, power fists, and all manner of things that go boom to obliterate both factions. I've got enough horrible thoughts in my head already, I don't need any more sticking in my brain like glitter.

I might be a coward, but if a game is too atmospheric I actually struggle to play it. I've always had to play STALKER in very short bursts and I'm scared to leave Skadovsk because of how quickly I get immersed.
I still remember the beheading scene from American McGee's Alice, so I guess that counts.
The mission in GTA: London where you steal a bus, pick people at a bus stop and then take them to a meat factory where they are forced into a machine and turned into sausages. Pretty fucked up

There's creepy children (the kids fucking torture the character you play as), really horrible elements that are physiologically freaky, horrific enemies, and implied pedophilia. Seeing such things in a new HD GTA game would be messed up (and absolutely amazing).

Random scattered things you're not meant to pick up on and are entirely different tone from the rest of the game. Also in Saints Row 2 There's a house with a slasher mask out the back and if you go near it you can hear a murderer running around the house and killing people.

Also that is my favorite ps1 game.
Megaman Legends and Legends 2 both royally freaked me out as a kid, particularly the ambiance of the ruins. I remember only getting as far as the alien spaceship... As soon as I realized that I was abducted and trapped in that ship and couldn't go back without pressing forward, my mind kind of went in full panic mode. As soon as the aliens started coming for me I quit.
