It mostly scared me because I didn't actually trigger it, it randomly just went off while I was taking care of some goons in the other room. Another thing in Hotline Miami that fucking scared the tar out of me is the fucking asshole in the van at the end of the eleventh chapter that drives right through the wall during the chapter clear portion with an incredibly loud sound effect. Not really something that disturbed me, but it really fucked me up.
This enemy in particular from Zelda: Twilight Princess didn't scare or disturb in any way but the sheer amount of tension between you and the hand makes it worse. The music starts to speed up the closer it gets and you panic when you see him come through the walls until you leave the palace with the light spheres in hand. Resident evil 4, I played it when I was 12.
The only thing that's ever really shocked me to the core in a video game was when I first saw the chainsaw death in RE4 [media]http://www.youtube.com/watch?v=cC3Gil_8-dA[/media] Something about how sudden and visceral it is seriously fucked me up for a couple days.
it's been said but Silent Hill 2 really fucking disturbed me as a child, playing it now it still freaks me out but I can appreciate everything a lot more.
The flood in halo 1. I remember the only way I got through the library when I played early on was by quietly cursing them out as I went through the level.
I remember there was a part in the original God of War where you had to push some guy in a cage to his death. He begged for his life to whole way, and I found that pretty hard to take at the time.
I played the demo for Sanitarium when I was a kid and only recently was able to play through the full thing last year when it came out on Steam. Dead Space 3, Isaac dying to the Puker, Divider or Swarm. [t]http://i.ytimg.com/vi/G8wAnp4JF94/maxresdefault.jpg[/t] The Puker should Isaac run out of health while grappled, it's acidic vomit burns through his helmet causing it to dissemble (His mining suit has a lot of very unhelpful safety features), the Puker vomits all over his face and down his throat leaving him to try and throw up the acid while his face burns off, collapses dead in a pile of vomit and his own flesh. It can leap and strangle him, should you not get it off it crushes his neck and decapitates him, then roots it's tendrils into his body taking control of his corpse and shambling off. Swarm are small gross tick, spider looking things. They find dead bodies, burrow into them and reanimate them into 'Wasters', which are pretty much zombies, have to rely on guns or melee rather having their limbs turn into blades or abilities like the others. Anyway should Isaac fail to shake them off, they start to burrow into his armor and hurt him, and flings his helmet off to avoid one that crawls in there. Another crawls out in front of Isaac who at this point is sore and slow, and forces it's way into his mouth and down his throat. After a brief moment of confusion he doubles over and throws up shitloads of blood, coughs for a while and stands back up zombified into a 'Waster' after the Swarm mutates his body from the inside out. Then of course there's the babies that have mutated into bloated explosive slugs, the toddlers turned into wall crawling spiders that shoot needles out of their intestine tendrils, and the young children turned into screaming flailing sprinters. And a lot more of enemies taking joy in abusing Isaac's head or mouth since his helmet comes off if damaged enough.
Fucking Silent Hill 1 man, the monsters in that game scare the shit out of me. Especially when there's that pterodactyl that flies through the mist and gnaws at your head like a motherfucker.
The mother fucking mannequins in Condemned Jesus fucking christ. I still have nightmares.
The first couple of resident evils and REmake give off a really good sense of dread, like you want to avoid all monsters if possible. especially when you realize that the zombie you shot a couple times in the chest awhile back is now up and running again as a red clad short sprinter with wolverine claws.
The doll room in Ib really got me the first time around. There's sheer sense of dread and hopelessness against an approaching danger and it fucking crawls out of the picture frame. [media]http://www.youtube.com/watch?v=quS2jNzbN4A[/media]
All the dreams in the 2 first Max Payne games scared the shit out of me as a kid. Come to think of it all kinds of insanity related things scared me back then.

I'm mainly talking about the cutscene introductions/endings for the characters, some more fucked up then others but all of them are fucked in some way or another. The Mr. and Mrs. Cutter part of Okami was pretty creepy to me when I played it.
