Peggle Extreme when I was young. I did not understand headcrabs on unicorns but I kept playing for the extreme fever.

I was playing as a little kid, during the night, eyes glued to the PC screen, alone. That shit freaked me out so hard I never finished the game until I was a teenager. The illusion of door safety was broken.
New Londo Ruins from Dark Souls. The ghosts looked so...wrong. Their unearthly shrieks didn't make it any better. What made it worse was when you drained the ruins. [sp]the whole ground was covered in a mass of dead bodies. WORSE still is that you make squishing, crunching noises when you walked all over them. There is also the large mound of bodies that's by the exit where the people made a sad attempt at leaving to escape drowning. Adding again the fact that they all look naked and pale just adds a cherry on top in this Silent Hill-esque sundae.[/sp] UGH. I'm actually amazed that I want Bloodborne to follow in suit of both New Londo and The Tower of Latria.
Also; Sweet dreams, as they have with mine.

Fuck this game. It's brilliant and horrifying. I got so scared when I played that I needed to pause the game just to stop my heart from racing so hard and fast.[/QUOTE]
God damn the bad end of that game is upsetting and uncomfortable.
That torture scene in the first bit of Splinter Cell: Chaos Theory. It was very tame compared to some of the other things in this thread but I was young and it just stuck with me.
for some reason i still remember exactly how the dude would scream
