Zoom in on the picture. You can see her mouth curves upward, a sign of enjoyment. Also look at her eyes. They are squinted. Another sign of enjoyment. If she was in pain or distress her mouth would curve downward, her brow would be furrowed, and her eyes would widen.
She is trying to get away, but at the same time she's enjoying it. Kinda like being tickled.
