I have no idea how to summarize this, so let's consider this as XBL Platinum or PS++ for the sake of clarity. The idea is to extend the current XBL Gold and make it a game-wide policy, instead of just for multiplayer. Instead of blocking multiplayer, the game can block the whole content if subscription expires. (Hold on to your pitch-forks, please)
The Idea: Publishers should sell the games on console's marketplace on day 1, just like steam. You can buy the game for 20$ which will come with 10hour subscription. After that each extra hour logged will need 1$. XBL gold currently uses real time to calculate your subscription, while XBL platinum will use gameplay time to calculate your subscription amount.
As soon as you recharge your subscription, you will have to download an online pass which will allow you to play the game. Since this pass will be downloaded on your console, you will not need any "always-on" internet connection to play single player.
Sharing with Friends: You will be able to copy the pass to a usb drive and take it to friend's console along with your gamertag. You will then be able to play on your friend's console for that stipulated period using only your gamertag. As you have already paid for that period of subscription, there will be no restrictions on using any gamertag.
Safety against hackers: Since the pass will be encrypted data like current online pass method, it will be pretty secure to hacks. In case if it still gets hacked, MS can push a system update which changes the encryption key, so that hacked pass won't work further.
Bandwidth Issue: Just like steam, games will be available for pre-download. In case, you are unable to download the game, you can buy a feeder disc with game data on it for 5$. There will be no subscription available for offline purchase. Even with feeder disc, game subscription will have to be bought online from console marketplace. This will see downfall in purchase of discs, and will help save environment.
Charges: 20$ for initial purchase(10 hour sub) 30$ for AAA game. 1$ per hour for extra hours. Single player only games can go for 2$ per hour, as they won't see much subscriptions in comparison. 5$ for a feeder disc.
Advantages: No piracy. Pay as you go. If you don't like the game, don't purchase further subscriptions. No used game sales. Able to share with friends easily. Publishers will have to work hard on games to increase subscriptions, quality of games will improve.
