Seriously though. I think the scariest game I played was Afraid of Monsters: Director's Cut. That thing scared the crap out of me. The atmosphere... the enclosed spaces.... the fucking zombie that pops up right infront of you.
Penumbra scared the shit out of me, but I really enjoyed that fear and it's a fucking great game. Up in the ranks with HL2 in my opinion!

It made me jump out and shout.
Aliens versus Predator 2 (The game, not the unrelated crappy movie). The motion tracker only served as a new way to scare the crap out of you. You never know if that *beep beep beep* is a ravenous alien, or a large beetle, or something blowing in the wind.

Dead Space in the dark with the door closed and the sound all the way up. That game has a brilliant soundtrack. Though it's pretty predictable. Whenever the lights go out there's going to be a few Necromorphs (I believe that's what they're called in the game) to kill. Very rarely will it be otherwise, unless it's a large, open area with several view-obstructing obstacles in it.
