This is actually something concept artists do, they have a character silhouette drawn separately and just add it to the composition for scale and/or detail, you can see it quite clearly on the borealis concept art for HL3/EP3 that surfaced a few years ago with the advisors.
Since the concept art is just a way to show the dev team what the game is supposed to look like, not advertise it, it's mostly acceptable to do this kind of stuff.

This concept art is incredibly surreal, but unfortunately CoD is never trying to be surreal. It's all about big grand vistas that act as set pieces for driving plot. I would love to see a shooter outside of Spec Ops: The Line that can take a moment to be surreal.

It was a high explosive grenade, he'd have a higher chance of surviving kicking it and blowing off both his legs than crouching and having the entirety of the concussive forces placed on his upper torso.

It seems he got very lucky with his positioning, by blocking it with his foot he spared the rest of his body and likely many of the others.

the 42's firerate was actually pretty smooth for recoil, giving out bursts from a position like that isn't that confronting.
