Gave me heart attacks as a 9 year old. That soundtrack still sends chills up my spine.
That "Stop and Go" level of Donkey Kong Country, with those superfast lizards.
deep water no matter even if i knew there was nothing there i always thought there was some giant monster or fish in there
That part in Donkey Kong Country 2 where you are in a bee hive, and all of a sudden are chased by a GIANT BEE and you have to run to the end of the level. The music didn't helped. At all I remember when I finally manned up and won the level. I felt awesome
Any water in any game
the witch from granny's garden fuck that witch

The security robot beetle things in one of Rouge's stages (Egg Quarters) in Sonic Adventure 2. Oh on the Sonic note, every one of Amy's stages from Sonic Adventure I just, couldn't stand. ZERO was just god awful scary looking to me, especially CHASING you, and the first freaky ass stage in Twinkle Park didn't fucking help. I didn't get the balls to play through those levels until I was much older and I still feel massively uncomfortable playing them, Anything that seeked you out and you were pretty much helpless when they find you really scared me in games when I was a kid. Especially if they were robots...

the fact in berzerk a face chased you scared the hell out of me
Going really deep underwater in Super Mario 64
Anything that chased me.
The first zombie in Resident Evil.
That music is just so eerie

"Hey boss, I just finished designing an enemy which is incredibly fast, only vulnerable when it's actively attacking the player, and never stops screaming at the top of it's incorporeal lungs after it notices something to kill." "Excellent! Put it in every small, cramped, area that the player can't retreat out of."
Episode's 2 and 3 of Doom. I was really young when I first played it, maybe 5 or 6 years old, and it was the first time I had ever really seen hell being portrayed in any form. Just every little detail that they put in was pretty disturbing to me at the time, from things like impaled corpses to walls made entirely out of intestines and flesh. I remember one specific part where there's a wall of people's faces moving that used to freak me the fuck out. Having replayed the game multiple times now it just looks kinda silly, because they all have dumb expressions on their faces, but as a child I remember them being pretty disturbing. [editline]6th August 2013[/editline] Also the part where you have to fight black ops in Half-Life for the first time. To be honest that part still unsettles me a little bit, just the way you know they're out there stalking you, and you're never really sure if you've finished them all off yet.
the sligs in abes odyssey/exoddus. the sudden change in pace of the music coupled with the "FREEZE" and gunshots is just really jarring and makes you go "OH FUCK OH FUCK OH FUCK". back then it was really scary but now i look at it as a really brilliant way to create a sense of urgency
Also, THESE FUCKING THINGS. Singleplayer Deathmatch was, is and will still be an impossible thing for me to do in Cube 2: Sauerbraten. For those unaware/those that never ever played Cube 2 (big mistake, do it, it's a really fun game) - these red things are Hellpigs. They are the weakest enemies in the game. They've got only 50 HP (for referrence, one pistol bullet hits for 25 HP) and they're purely direct combat. Can't hit you on any range outside of melee, extremely weak, so why are they such a big deal for you? - you might ask. It's in their look. Their fucking look. I don't know why, but for me they're just super disturbing. Also, the FOV in the game can be set to a different amount by every player, but my most "comfortable" FOV for Sauerbraten was set to the amount that when Hellpigs were attacking, they covered 85% of the screen. Like, you know, the entire center of the screen. Imagine it popping on a screen for a kid that isn't aware he's up for a slasher game ("hey, making maps in game, that's cool!") and you've got yourself a scarring for life. And the fact that Sauerbraten's community isn't really convinced that 'Pigs are already scary isn't helping.
I was scared shitless of xen and the resonance cascade in half life. Aside from that, underwater levels.
The butler yes, THAT one
the security robots in every fucking game ever seriously what the fuck i'm like 8 i'm not here to get a free heart attack
The whole game... I can't explain it... it was too bizarre, the characters were weird, the sounds the caracters made were scary, the music was scary, the ugly skybox that was about to fall on everyone heads... it was just damn scary!
The alien-dummy from Unreal tournament's practise room or whatever.

Skeletons for some weird reason, just always found them creepy, scary and unnatural. [T]http://static.giantbomb.com/uploads/original/0/1481/560387-medievil.jpg[/T] This fucking guy is a good example. Another fella that scared me as a kid... [T]http://24.media.tumblr.com/tumblr_lfjuo34E3w1qb8xlbo1_500.jpg[/T]
The canals, Ravenholm and the tunnel part on HL2.
This. I didn't give a shit about dead scientists, blood and gibs all over the place, headcrab zombies and ichthyosaur but the Valve logo scared me.
Thank god we didn't have the new intro back then or else I would've never dared to play HL2.
The power going out in Luigi's Mansion. Yes, I was weird, never got scared of the game though until that part. Before and after, everything was good, but for that one bit I was scared shitless.
