Fallout /is/ my ideal game. But sometimes I wish they fixed the crashes, made it look good without having to inject a shit ton of ENB crap, and make the animations much more fluid.
A game like the elder scrolls where you can never run out of unique quests, but that's a little impossible because that would require constant new mapping, dialogue, etc

"The Iron Throne" a Game of Thrones RPG.
Basically what Shores of Hazeron was, but better. A great big FPS empire-building game where you can interact with your empire at any level you like, from picking the rocks for building stuff right out of the ground yourself, all the way to commanding fleets and managing the industry of fuckloads of solar systems. I was particularly fond of boarding opposing ships, so being able to do the same but with better controls and more destructibility would be amazing.

Sorry for the late response, but the first thing that came to my mind was the Legacy of Kain series.
A huge 1:1 scale of earth survival-looting-killing-building game with the most realistic physics possible and the ability to withstand an unlimited amount of players, along with practically everything you could dream of in a survival game -- i.e. vehicles, fully customizable bases, anything for a weapon, etc.
And a driving arcade/sim where you could also travel a 1:1 scale of earth and just explore and do fun stuff with cars. Of course there'd also be realistic physics and whatnot.
Interesting how many people want unrealistically huge openworld games.
I mean yeah it would be awesome if there was a humanly possible way to make them consistently fun, but huge openworld games tend to be shallow as fuck because the devs simply can't afford to pay attention to every detail and to everything there is in the game to do. It also means that they have less control of what the players do in the game and can't communicate their ideas that well. The player gains some "freedom" in where they go, but in the end they always end up doing the same stuff over and over. It's inevitable.

Last I checked Garry's Mod was horrible for having a lot of vehicles spawned at once, but one can dream. I guess for RTS you wouldn't need such an accurate physics representation, something along the lines of Robocraft would do.

It'd be a cross of NFS Most Wanted,Street Legal Racing Redline,Forza Horizon and Gran Turismo.
You can own your own car and modify every little bit of it. You can open the hood and replace every single engine part,or replace your interior parts. You can have different jobs,like pro racedriver,rally driver,truck driver,cab driver,street racer,police pursuit car driver,car reviewer (like Top Gear) etc.
There'd be toggleable options for how advanced the game'd be. You can toggle wear on your car,random breakdowns and fuel consumption.
You could go to car shows and show off your car. You could go to trackdays. You could fuck around with the police. You can travel around the world. You can start out with a second-hand rusty Golf and end up with lots of garages filled with the newest supercars and concept cars.

I was on the can, but my idea for a game would be something of an RPG-Brawler maybe call it Dances with Toons, Fight of Champions or maybe Toonabusta Supreme (with cheese!).
Like take a dash of River City Ransom, a dash of Streets of Rage and possibly either Skullgirls or Castle Crashers. the narrative of the game is something similar to Saturday Morning cartoons crossed with live action TV shows, were essentially Cartoon characters are coexisting in a world, where cartoon physics is something of a religious practice that actually exists. (so The Force, Wuxia Karate crossed with the [I]Space Jam[/I] universe)

• [B]Stanley Porkin(s)[/B], the pig-like son of the esteemed Constance Porkin(s), goes to school as a second-generation french exchange student. maybe his girlfriend is kidnapped, his combat persona is [B]Porque Chop[/B], a karate/sumo-themed brawler, his animation style would be reminiscent to Song-a-long Disney Shorts.
• [B]Cinzo Alberta[/B], an (clearly human) amnesiac foreigner who arrived at the Town of Adventure for some unknown reason and enrolls at the high school as the chummy upperclassman senpai PE teacher, seems to know more overall but seems to have suffered a mysterious accident, in combat he assumes the form of [B]Stanza the Bloodless[/B]; his animation style is reminiscent to gritty 80's Japanese-Italian OVA Anime, but because he was Bowdlerized, most of his mannerisms and speech doesn't match with what he does, his combat style is sci-fi modern, where regular guns fires lasers but because of censorship, the flashes of light functions as an extended hitbox. (Cinzo = Kinzo, just old K = C thing)
Bad
• [B]Morpho[/B], a (non-sapient) Dog near the Domillite Meteor crashsite, warped by cosmic radiation, he has been uprooted and mode-locked into the psychotic [B]Stubby[/B], his animation style is just basically Ren and Stimpy whilst electing for a more chaotic, violently graphic combat style, like jumping into the air and shapeshifting into a house boat, or just smashing teeth out with an Oar.
• [B]Igor Jones[/B], a diabolically charismatic Vampyre Duck, though the lack of body is of no convenience, the robot body he inhabits is called [B]Old Joe[/B] of which he sits inside the chest cavity, his moveset consists of boxing and alchemizing his hands into bladed talons, with the occasional head-toss and vapor streams of blood from his eyes. the animation style is a mix between Manga and motion comics. like Cinzo, sound effects are emphasized as hits, thus timed-explosives may cause scratch damage just for shooting past people. said to have prior connections to Constance Porkins.
The modes could range from the Generic Beat-Em Ups which is straight forward like Streets of Rage where character choice doesn't matter with midpoints and transitional challenge paths. the story mode where you play as single characters represented as a mapscreen interlaced with Boxes similar to Wario Land 3's Stage Select or Super Smash Bros Wii U's event screen. and the 1V1 mode where the planes are forced to 2D for a Street Fighter-like experience, or 3V3 to tag in characters for Supers/Hypers.

Metal Gear Solid V with Loadout-tier weaponcrafting, Garry's mod-tier creativity options,hitscan weapons, great music, an intricate and interesting plot that fits within Metal Gear, co-op, a huge world, satisfying weapons that don't sound like peashooters if you don't want them to, a complex and satisfying CQC combo list, lethal or non-lethal options, being able to do things that don't shit on the canon, good KB+M support, good optimisation so that I might actually play it, and tackling taboo subjects, like alcoholism, depression, killing for a cause, or losing touch with oneself.

That's a thing that exists and is free: Dwarf Fortress.
Republic Commando but with Rts-like elements and an open world.
Build small teleporters to bring in units, invest in things to allow stuff like drop pods/dropships, that sorta thing.
Eventually Mount & Blade large-scale combat.
GTA with a prologue where you take a teenage character, put him in some horrible situation that gives you a choice to kill or not kill someone, either way you're framed, you go to jail for over a decade but play through scenes in the jail that afect the type of character you are at the point of your escape and also later events in the game and contacts available to you on the outside. These scenes could be:
Passing the shower block to hear someone being beaten by a gang. You can walk on by, help beat on the guy to get in with the gang or do the impossible and fight with him against them, being almost impossible to win, you don't die but you make a lifelong friend who will help you out compared to the gang who on the outside will use and abuse you but give you access to a lot more firepower and backstabbing. You could go to the prison yard and work out and in 10 years your character will be buffed out, etc.
Lots of big decisions that impact the game. With stuff like GTA it really isn't that hard, theres relatively few cutscenes to gameplay so having differences wouldn't be impossible and it lets you decide if your character is an asshole who commited cold blooded murder or someone put in a bad place forced to become what he has no other choice but to be.
You can drastically change the game making more loyal friends as you slowly move up the ranks of power and build an empire or have much force much earlier on but never be sure who might betray you for simple monetary gain. Of course your loyal friends can be tempted to but its less likely (Scripted obviously) and they will help you where others would not. You can also trust them to run your organisation.
The differences open up different missions with of course a core series that always appear but some might be sub-plot arcs related to the character or organisation that you can't get without following a certain path.
My friend and I thought it'd be funny to have a sports-fighting game, for example you have something along the lines of golf, only you're not taking turns on your own shots, everyone's going after one ball trying to be the one to get it in the hole, while trying not to get beaten up with golf clubs by other players/fighting other players with your own golf club. I think it'd be a fun/funny multiplayer game, just the stress of trying to line up a good shot from 10 feet away while you hear the other players running up behind you.
combination of dark souls and armored core
You're a self-aware machine in a once-utopian megacity, its human populace mysteriously gone for some thousand years or so. You scavenge parts to upgrade/equip yourself so you can survive against the city's less-than-friendly inhabitants. You can encounter other players at random and without notification, and whether you try to ally with them or kill them is purely up to you. Parts can get individually disabled similar to Metal Arms.
I'd really like a WW2 game on a modern engine, like Frostbite 3 or something.
