Media Player Classic on Windows, VLC on Linux (Songbird is cool, but VLC plays more stuff). As for Mac OS... I'm just shitting you, I couldn't give less of a shit about Mac OS.

I tried using Foobar once but skinning it is such a complete hassle that I just gave up after a while.
Winamp because I haven't yet been arsed to download XMPlay.
Also Winamp because it's got iPod support
Spotify for the most part, but I keep iTunes installed for the iPod.

I import all of my music into iTunes for my iPod Touch so I have no use for any other media player besides Windows Media Player.

Why? It's slow as shit, ugly, the library doesn't autoupdate and it's so horribly bloated it's silly. I only use it because it's the only way apple allow you to put stuff on your iPod touch, and even then I try to use SSH and iFunbox as much as possible.
I use spotify for music or otherwise I just use wmp. For movies I use VLC, I don't really like vlc for music though.
