Realistic animations, such as Left 4 Dead. Right now they're so stiff.
An addon that allows metropolice/combine soldiers to use custom NPC weapons properly would be fantastic.

I'd like an JC2/GTA style "action sandbox" map/addon that has citizen/friendly npc's that move around and generally make the map actually look alive, and enemy NPC's that patrol/protect certain areas, as well as a "heat" system that causes more enemies to spawn and find you if you provoke them. It'd be amazing if the citizens would react when you cause a scene.
An addon to allow you to select a object, even if it a effect object that has animations, go through its animations list, and select one to play when you hit a button on your numpad, better yet if it also had a wiremod version. Oh and the ability to select to loop it or not.
Not only useful for machinima, but could be used in rp servers.

My dream addon would be an "auto-pilot" admin. Something that could monitor what players are doing. I.e if it detects someone spamming, it will stop them and give them a warning then if the player continues to spam it will kick.
Monitoring of chat spam, prop pushing, spawn blocking, death matching, kicking of idle players etc... would also be useful.

To answer your question. Use the screen under Useful & Explosive then place an RT camera.
My dream addon would be one that spawns a generic pistol or rifle mag prop depending on the holdtype when the npc is reloading and when they die the writhe around a bit like in splinter cell conviction.

RP scripted npcs are really awesome for that purpose
Citizens work around and run if they hear shooting, cops attack you if they see you attacking someone, gangsters attack cops. And they all respawn when killed, keeping the population the same
I remember putting tons of citizens on Bigcity and going on a car rampage
So fun
