Something like Scars, but for trains, as making a train and getting on the tracks can be a damn pain in the ass.
A tool that let's you record or download sound files and make NPCs say them WITHOUT needing SDK.
As many said, realistic water and air. Would make building fans a lot easier, and would allow for neat water based contraptions.
Maybe allow heat to convert water to steam to make working steam engines.
And a dismemberment mod that has actual dismemberment with blood, not just going splat or losing a limb with no blood.
A turret addone where you click a vehical seat and it becomes mouse aim. That would make my life.

My dream addon would be a way to make it so no low lives would EVER GET F****ING ADMIN!!!!
That happens way to much, they just suck up to one admin, become one, and constantly screw the game up for me...

Plz help every time i open gmod it will open fine but it will say creating render files i have been there for a while and it wont work.. .but the thing is i can still hear the buttons in the game sound.. So like when ur cursor goes over a button it will make a noise well same with mine its like its stuck on that render file thing someone help

Yes.
A marching addon, so that you could make npcs walk together in unison, all perfectly spaced between each other. Oh and a fully controllable Scarab too.

There is the NPC's that can enter cars part, but it's on a map. I forget what its called. I think it was something like botmap? I dont know but check it out, you race the NPC's.
I don't think It's been done already, but the sort of Add-on I'd love is one that made it possible to control your player-models' head movement like you would in ArmA II (or ArmA 1, even though I've not played it)

My dream mod Pulsareffect, is my opinion.
A modular weapons system. You get out your toolgun, select which weapon you want to spawn, select modifications, (rate of fire, accuracy, etc.), and select attatchments for it. Then you spawn it and walk over it.
That would make my day...
An addon that allows you to instantly change the gamemode, update installed addons while in-game and an Ingame SWEP Maker.
