Yeah, more player models, tired of living on "The Island"
Completely dynamic event system that is independent from players.
Alot of these ideas are good with populated servers, but they need to give you something to do when you are alone, or have little population.

TTT wasn't really an idea. The first concept of it was called "Murder" by Gmod4ever.
easyier commands and instructions, also if someone create a medieval or other different roleplaying system, it could exsist a "time" system, like when a certain amount of time passes (decided by the server's admin) it could advance to another age, like, from cavemen to roman empire, from the romans to medieval, from medieval to modern, from modern to futuristic, and from futuristic to other three futures (decided by vote): apocaliptic (fallout style), steampunk or empire (star wars-"ish"). and from one of the three options to apocalypse or something else.
no guns
a simple and easy House and sleep/hunger system, also a inventory and resource systems, like on the survival gamemodes sleep: get a bed entity and use it, or find somewhere comfortable and use some sleep command, hunger, buy or find food (hunt or farm/search for fruits/herbs/vegetables or create animals for fur, leather, milk, meat or anything else), resource: use it to create itens, weapons, tools, buildings, etc. house: get resources and build one, inventory: store weapons, food, resources, etc.
I'm currently developing a role play gamemode of my own, and I'm happy to see that some of my ideas are being mentioned here.
Anti-Drama system.
Whenever drama is started all participants are instantly slayed.

A severe lack of guns, some major nerfs and buffs to guns, however you can use your fists and such to knock people out. And some makeshift weapons like lead pipes and nailed boards.
People won't join servers without guns, sadly.
Guns should be available if you want to keep the server active these days.
They can be hard to get but if you can beat a guy to death with a lead pipe, a pistol should also be possible to get.
You could try a forging system with a chance for you to mess up.
Example:
You take the lead pipe and melt it down.
You use the metal to make the shape of a barrel and such.
You eventually use it to make a low quality gun or the gun becomes mis-shaped and becomes unusable. (60% of failure?)
Materials could be hard to find and on top of that you could have a chance to mess up, resulting in the gun not being created.
The more successful guns you make, the better you are at using the materials and the higher chance you have to not mess up. (This would take a while though seeing as it is serious RP. You obviously don't become a professional in 1 day.)
Using this skill, you have your player job and can sell guns. (Even though it is probably illegal)
This is just an idea to have guns available in Serious RP. After reading through it, it may sound a bit like Stranded. :P
They will leave because it's too complicated, or takes too long.

Because in real life, everyone knows how to make guns.
in soviet russia gun makes you. so everbody is a "son of a gun"
How about you implement NPCs to a further degree? I mean, why don't you add an environment complete with birds, fish, people etc. You could Kill two birds with one stone; being able to give the player something to kill while being able to protect or harvest for supplies from.
You also need a quest system with basic things to keep people occupied for a while.
I'd also like to see server to server traveling which would require servers and a dedicated lua scripter and hammer mapper dude-team.
Other than that, I'd like to see some aesthetic things such as weapons/armor being shown ontop of player model, some gestures and stances along with some grab and some item interaction anims.
Why don't you make the maps infested with NPCs warping around and lag levels jumping to the moon.
