Looks like you haven't got mysqloo installed, fusv_mysql.lua requires it, won't initialize the RetrieveData function without it.

Also needs a database, see the wiki on the github.

I still had problems with this before and after using MySQLoo 8 and 8.1 also tried using tmysql 4 with the MySQLoo Wrapper and had this issue persisting. I just gave up in the end after trying to get some assistance with this addon.
