Whilst I've hardly tried any mices at all the Razer Lachesis is pretty good and meets all your criterias.
DPI sensitivity change by click of a button right below the scrollwheel. It's served me well for a good 3 years now.

I thought the same thing with my Copperhead. Then I got the G500 on black friday and realized how cheap of a mouse the Copperhead was.

the difference between my G5 and MX518 was phenomenal
the only problem I have with it is that the wheel wears out quickly and 400 dpi is a bit of a hassle for desktop use @ 1920x1080

I don't see why people make threads like this, just get something that feels comfortable to you. 99% of the people on this forum don't play any games competitively so why does it matter? you aren't going to make use of that slightly better tracking anyway
Just borrowed my brother's MX518 to see if all the hype about it having a better sensor than the G500 was true. It's not.
I tested both as fairly as possible by using the same surface (Razer Sphex), 1100 DPI, default Windows speed, acceleration fully disabled via registry mod. The game I used was 'osu!', a rhythm game which requires extremely fast and complex mouse movements, more so than FPS games at any level.
If anything suffers from negative acceleration it's the MX518. Flicking from one end of the screen to the other repeatedly several times a second (as is the kind of movement necessary) was completely impossible - it always fell short or did some crazy erratic movement. Shorter and slower movements fared slightly better, though they did feel 'heavier' than the G500.
So this leads me to the conclusion that for those who use a mouse with wrist/finger movement and never lift the mouse or move their arm - and use a high DPI to suit - laser is superior. For lower DPIs I cannot say anything, but everyone seems to be convinced optical is better in that regard.

A4 Tech X7, nice texture, mine has changeable LED colours but the one on that website doesnt, and it has a triple-click button (handy when using pistols on games, one click = 3 in-game clicks.

The G5 stands in between the two and the Storm Sentinel is completely pathetic. Those are the only 4 mice I've played osu! with.
I'll see if I can find an old intellimouse lying around sometime, but it's probably not going to be easy to scale the sensitivity to match my muscle memory for 1100dpi, so it wouldn't really be a fair test.
Also I wouldn't exactly call the G500 mediocre - not once have a I missed a note where I can pin the blame on the mouse's tracking performance. People play this game with pen tablets; the tracking has to be pretty much perfect, and it is.
