for what operating systems? as far as I can tell, anything below Win10 still works perfectly through UXTheme Multi-Patcher. the November update (1511) did indeed break shit under Win10, but deviantart is slowly but surely pumping out themes to fit the new code.
in terms of theme patchers for Windows 10, i don't know if there's a good universal one that supports it. i would suggest installing whatever UXTheme patcher the theme you're trying to use recommends

I went for a simplistic Windows 98 look mixed with a Commodore 64 font and ended up with some Rollercoaster Tycoon-ness in there as well.
