It's the Logitech G15 (second revision which has a third less programmable keys) and I bought from some mom and pop computer store in Summer 2008 while in Red Deer Alberta.

I actually saw the LACK rack before I had my servers. I knew as soon as I got them that I wanted to use a LACK. In hindsight, however, I should have gone with the black model. It would accent better with the surroundings.
