Its hardly delusional since it is MGS were talking about, and shit is lining up in really eery ways. If it was ANY other game series and developer I would be 100% with you, but this is fucking MGS were talking about and Kojima. Plus if you were involved in this, so the Doctor, you would deny your hardest if people asked anyways. No one part of this would be like "ahh you got me :)"
Like I said, I think its more Kojima purposely making these little connections since he could have easily gotten inspiration from this doctors work at some point in time and wrote parts of MGSV based off his shit so just left little shit maybe as a connection for a homage?(the 2012 cyprus map and TedX Limmasol, and anagram are still weird as fuck as the Ted X is happening in 2015) This doctor did not just fall out of the sky this year, hes written successful books ages before this.

MGS has a lot of history to back itself up with anyways. Its not like its some dev just took his face, because the actor already looks like the Doctor as well. You also cant sue someone even if Kojima took inspiration from the Doctor himself, both in looks and research. Unless Kojima straight up word for word copies shit the Doctor wrote then theirs not much in this regard either if somehow shit the Doctor wrote ends up being used for story shit in MGSV.
This whole situation is just weird as fuck. Some of the shit the Doctors said, the likeness, the MGS connections, this entire head transplant shit coming essentially out of nowhere and making global news, the anagram, Kojima getting fired, using "Heads up" in multiple emails from the Doctor. The Doctor jumping immediately to trying to sue Kojima faster than an American that spilt coffee on themselves.

That is the one that really sticks out to me.
I go back to my previous remark: both stories are true. The doctor is obviously a real legitimate doctor. But the connections are just so numerous, its honestly hard to believe its only coincidence. Kojima did say he was doing something taboo, and I recall he said that if it worked would be basically the pinnacle or his career or completely ruin him if it failed. So to me it seems like several years ago Kojima met this doctor or heard about him then met him, they talked back and forth for a bit about their work, then they basically agreed that Kojima could provide a patient and funding for this doctor's work and the doctor would help him by being part of his massive ruse. If the transplant is a success, Kojima will become famous not just in video games but in the medical community and even broader. If it fails, he may never hold another job again for contributing to a man's death in a fruitless operation. I'd say literally playing with life and death for marketing is pretty taboo.

Even without the Patriots existing, the entire AI speech at the end of 2 holds even more truth today. Its only getting worse as well. Its not as extreme as how MGS2 paints it in its context but anyone with a shred of self-awareness knows just how real the shit is.(not to say Kojima is a super genius who called this single handed, he just called the digital age in a fucking video game in 2001, this type of shit has been done by a lot of people throughout the years, Brave New World and Fahrenheit 451 are examples of similar themes but obviously not to do with the digital age)

If it helps, now this is my own personal long stretch theory.
The patient who offered his body is legitimately terminally ill and is going to die soon. Hes some guy in a wheelchair and iirc he is actually a programmer and loves MGS. He was asked on twitter, I saw screencaps and shit but the name is completely slipping me right now so im sorry.
Anyways, my theory is this terminal ill guy who is 100% dying soon might have had one of those "Make a wish" foundation type deals, I highly doubt such a thing is only for kids, im sure theirs other orgs.
