We've dropped unarmed nuclear bombs on and off the coast of our own country, and two on Spain. All of them because the bomber was crashing and had to jettison their payloads. We've also accidentally flew a nuclear bomb across the country not long ago. We overlook nuclear warheads all the time. Considering there were multiple armed Peacekeepers at this time, some of which ready to go at a moments notice, it's not unachievable that someone might misplace the test missile with an armed one. Though I'd assume that they would check and recheck then check again to make sure that wasn't the case.
