You know, research has shown that binge eating is neurologically extremely similar to alcohol or tobacco addiction.
If the guy managed to get to 1,000lbs then that sounds like a serious mental issue, and I wouldn't expect him to get better by 'just making right decisions' any more than I'd expect a binge drinker to just stop drinking. It can be done, but neurological dependency is a bitch and willpower alone probably won't work. The guy needs help.

i have the same sort of habit, though not to his extent
eating feels good to a lot of people, and for some that good feeling is one of the few they ever get, so it can be really easy to fall into that cycle and really hard to climb out of it

Since eating food activates the reward center of the brain, it can be very easy for someone to form a compulsive behavior around eating food. When you ask a drug addict why they go back/relapse and binge you will mostly hear an answer along the lines of "I was under stress and wanted to alleviate it" or "I found myself in a setting similar to my drug taking setting which induced a compulsion to use." This is comparable to compulsive eating since you hear "eating makes me feel happy, I eat when I'm upset/stressed, I eat because well I just have the urge to." That urge is very powerful, its a primitive part of your brain overriding all of your judgement shifting your perception to accept the urge or compulsion which is why in this video you can see how the person even justified/brainwashed their eating habits to her sister. Compulsions are very hard to break since they are very reinforced built in associations that you are trying to change from the outside. Since eating is usually done at home, the surroundings in which you eat then reinforce the behavior, almost like familiarity or expecting an outcome. If a crack head always picks up his product at a certain city block, even when he passes it when he's not buying he will feel the urge to use. Now imagine having an eating disorder in your home, everywhere around you are reminders "hey the kitchen is right there, why not make yourself something, ease that anxiety" it will take just as much work to remove the conditioning as it was to create it in the first place. Sure some people try to blindly justify their unhealthy eating, but I feel like it should be noted that those people are in a state where they have realized maybe they don't have control over themselves and that prospect to humans is very fucking scary.

There would probably be potential for serious complications during surgery based on how unhealthy the poor guy already is.

Contrary to popular belief, there is no such thing as "my genetics made me fat".
There are some genetic traits that [I]allow easier weight gain[/I] but it does not "make one fat" like how people say. And those with that trait are perfectly capable of watching their weight like any other person.

and there are also people with mental illnesses like depression that make it nigh-impossible to even have the motivation to even TRY to lose weight
I feel bad for him. He obviously has a serious mental condition that no one seems to be willing to treat him for.
