There's the chance they'd be pressganged into some sort of Volkssturm arrangement when NK civilian centres are within reach of NATO forces to liberate, since any military-fit people would already have been drafted into the military at that point. One could try airdropping food aid, but i'm not sure how effective that would be in mollifying the populace (especially since the NK military would claim first dibs on pain of death)
